I think this quote means that your conscience is always there, you know what's right and what is wrong.
If you choose the wrong you are going to feel bad maybe no one cares about what you do but you know you do care about yourself and if you don't, then that's sad because it is your life and you only have one, don't waste it. And if you want to have a clear conscience you have to do what you think is right. Your inner peace is the best feeling ever.
